What are the goals of the CPSP Program?
uTo improve the outcome of every pregnancy uTo give every baby a healthy start in life uTo lower healthcare costs by preventing catastrophic and chronic illness in infants and children
CPSP Services include:
services postpartum assessment, interventions, and follow-up services in:
uObstetrics uHealth Education
uPsychosocial
Services genetic screening, dental care, family planning, and pediatric care
Medi-Cal provider number and are in good standing with their licensure board:
uPhysician (obstetrician/gynecologist, family practitioner, general practitioner,
or pediatrician)
any of the following practitioners to deliver services appropriate to their skill level:
– At least 18 years old
Contact the CPSP coordinator at your local health department for an application. To reach the Pasadena Public Health Depertment CPSP, call (626) 744-6091.
Call the Health Access Program (HAP) Line at (800) 257-6900 if you need the name and phone number of the coordinator in your county.
The completed application will be reviewed by the local CPSP coordinator and submitted to the State Department of Health Services for final certification.
