Case Management Program

 

Your health care provider has decided that you may have a need for Public Health case management services. A Public Health Nurse (PHN) will provide case management services to help you get the care and services you need. Services may be free of charge*.

 

Once the referral is made, you will be contacted by a Public Health Nurse (PHN) who will complete the following:

An interview

With you and/or your guardian to find out what your needs are.

These needs may include:
• Medical Services
• Physical Needs (food, clothing, etc.)
• Housing / Physical Environment
• Family / Social Support System
• Vocational / Educational Needs
• Training Needs for Community Living
• Social / Emotional Status/ Mental Health Services

Develop A Plan

The PHN will develop a written plan with you of how to meet your needs and how often the PHN will need to visit you or your family.

Give Referrals & Resources

Depending upon what your needs are, the PHN will help you with finding resources to help meet your needs.

Offer Assistance

The PHN will also assist you with getting to services, including arranging appointments and/or transportation to medical, social, educational and other services. The PHN can also arrange for an interpreter.

Answer Questions

The PHN can answer any questions that you may have about your care and/or diagnoses, to help you better understand your medical care and treatment.

If you have any questions, please feel free to ask your healthcare provider or counseling nurse.
 

*Targeted Case Management (TCM) fees are determined by an eligibility screening.
