massage technician permit


What is the process to obtain my massage technician permit?
 

The City of Pasadena does not issue massage technician permits, it issues a tax permit through the Business License Office. 

The Police Department provides clearance and approval for the tax permit after the applicant and document processing has been completed. 

The documents can be picked up at the Business License Office or on an appointment basis at the police department.  Call Evelyn Quinteros-Alejos at 626 744-4508 to schedule an appointment. 

    Police Department application – 

  • Read the first page of the application – do not sign or date this page.  Police department personnel must witness your signature at the time you submit the application.
  • Read and complete pages 2, 3 and 4

Massage Technician Application Questionnaire – 

  • This form to be completed by five bona fide residents of Los Angeles County. Relatives, friends and co-workers may complete this form

Authorization to Release Information - 

  • Read, but do not sign or date this form.  Police department personnel must witness your signature at the time you submit the application.

Background Investigation Discovery Waiver - 

  • Read, but do not sign or date this form.  Police department personnel must witness your signature at the time you submit the application.

     In addition to these forms, the police department requires:

Current California identification – identification in the form of a California driver license or California identification card.  Either identification must have your current address.  California Vehicle Code Section 14600(a) requires anyone who moves to or obtains employment in the state to update his or her State-issued identification within 10 business days.

Photo - of the applicant, a head and shoulder color photo at least 2”x2”.

Doctor’s Certificate – stating in writing that the applicant has been examined and found to be free of airborne communicable disease.  This certificate must be signed by an M.D.  A certificate signed by a physician’s assistant or a nurse practitioner will not be accepted.

School Certificate or Diploma and Transcripts – issued by the institution that provided instruction in the field of massage.  The school must be a state approved institution.

Once you possess all the required items, call  Evelyn Quinteros-Alejos at (626) 744-4508 to schedule an appointment.  At the time of the scheduled appointment you will submit the required items then go the police department’s records section to complete the fingerprint process.  After we receive the results of the fingerprint process you will be called to schedule an appointment for an interview. Immediately after the interview you will be sent to the Business License office to apply for the tax permit.
