AN AFIX PROCESS

Improving Immunization Coverage at the Provider Level

What is AFIX?

The AFIX process is a CDC developed and endorsed quality assurance tool for assessing and implementing strategies for improved immunization coverage levels at the provider level. AFIX itself is an acronym for a set of strategies focused on attaining the primary goal of improving immunization rates.

  • Assessment of a provider's vaccination coverage levels. Immunization coverage levels and delivery patterns are evaluated by trained CQIS staff at the practice level via chart review. CQIS staff will conduct the assessment on site with the use of the Clinic Assessment Software Application (CASA) program along with the Quality Assurance Review (QAR).

  • Feedback of information and recommendations for improvement. Following the chart assessment, a one hour feedback session is scheduled by CQIS staff with the provider or other key stakeholders/staff. Strengths and weaknesses will be presented during the feedback session and recommendations for improvement will be discussed.

  • Incentives for providers to improve vaccination levels and to recognize providers' participation in the process. CQIS staff will provide quarterly follow-up and ongoing technical assistance with participating providers. Providers will also receive immunization updates, free resources, and staff trainings.

  • eXchange of information among providers relating to performance and strategies that have or have not worked. Through information exchange, providers can reinforce their strengths and make recommendations for change in their own practices. Providers will also have the opportunity to identify where they rank with set standards/goals with regard to immunization coverage levels.
